We've seen Sam and Dean fight vampires and exorcise demons and it would not be a surprise if they'd have an imaginary friend now, would it?
Have you ever had a friend that believed in everything you say? A friend that gives you advice and listens to your deepest darkest secrets? And this friend promises not tell anyone at all. Well, he wouldn't especially if no one can see him. Except this time, said imaginary friend decided to ask for help.
Sam's imaginary friend Sully, played by Nate Torrence, decided to come back after a long period of absence since Sam's parentless childhood days and needs their help because he's in deep trouble. Of course, Dean is very skeptic about all of this but when Sully tells Sam that his co-imaginary friends are being murdered one by one, something has to be done, as explained by the show's director Jared Padalecki.

Jensen Ackles added that "Just My Imagination" (the title of the upcoming episode) ".. is really fun." But Ackles did share a spoiler, one that tells you that it's not safe to be with the Winchester Brothers.
"When Dean walks in and sees Sam talking to nobody and says 'What are you doing, who are you talking to?', Sam's imaginary friend reveals himself to Dean for the first time", says Ackles, relaying his favorite lines of the episode. Sam introduces Sully, his imaginary friend. To which Dean aptly responded with "I'm gonna get my gun." Good ol' reliable Dean.
Padalecki says that Sully acts like Sam's sort-of Castiel, as told by EW.
This episode is a great opportunity to get a look at Sam's past, a childhood and a side they have never seen before. You know what it's like talking to an imaginary friend right? You have your own little world? Yeah, you get it.
